COMMUNITY SUPPORTS - Provides various levels of support to individuals who are living on their own or with their families. Included is in-home respite services, family reimbursement, socialization and organized activity opportunities as well as site-based programming. Reimbursement obtained per specified guidelines. DISABILITY INFORMATION AND SUPPORT - Assists individuals with developmental disabilities and their families in securing eligibility for Office for People with Developmental Disabilities (OPWDD, formerly OMRDD) services. Also provides assistance in accessing the various services from the agency and provides referrals to other agencies within the community that may be able to offer assistance. No fee. Call for an intake appointment. Eligibility: Must have a documented developmental disability and be 3 or older. GUARDIANSHIP SERVICES - Assists individuals with disabilities and their families in the preparation of guardianship petitions. Can act as the primary guardian, standby guardian, or alternate guardian. Guardianship can cover DNR orders, health care proxies, supplemental needs trusts, wills, financial planning, and special needs planning for parents of children with developmental disabilities. Must have a documented developmental disability. OFFICE FOR PEOPLE WITH DEVELOPMENTAL DISABILITIES (OPWDD) RESIDENTIAL SERVICES - Offers supervised residences for individuals with developmental disabilities. Homes are inspected and certified by OPWDD. Services provided are based upon each individual's specific interests and needs. Reimbursement obtained per specified guidelines. STEUBEN EMPLOYMENT SERVICE - Provides intensive training and work opportunities to individuals with developmental disabilities. Both vocational employment and supported employment opportunities are available. Employment Specialists and Job Developers support and train individuals in resume writing, interviewing, and obtaining employment. Job Coaching is available to teach on-the-job skills, help individuals maintain their employment, and give support with work-related issues. Call for information. Call for an intake appointment.
