PATHWAYS 33 Denison Parkway West, Corning NY 14830 (607) 937-3200 COMMUNITY RESIDENCE FOR CHILDREN & YOUTH - Offers two residential homes for children who may otherwise be placed in psychiatric inpatient or residential treatment facilities. Services provided improve and enhance their social, emotional, and functional behaviors while enabling them to live in a community-based setting, attend local schools, and participate in the cultural and social opportunities of the area. Staff works closely with the child's family in order to help them return to a more normative living environment as quickly as possible. Eligibility: Must be 5-17 years and at risk of psychiatric inpatient or residential treatment. FAMILY INTERACTIONS - Offers visitation services to non-custodial parents of children in foster care in a safe, secure, home-like atmosphere to promote positive relationships and work toward reuniting families whenever possible. The program operates in collaboration with the Steuben County Department of Social Services. Set fees. Call for an intake appointment. Eligibility: Must be referred by the Steuben County Department of Social Services and have a child in the foster care system. FAMILY SUPPORT SERVICES (FSS) - Provides short term respite care for caregivers of individuals with developmental disabilities, recreational activities that are age appropriate and take place in the community including Ride with Pride which offers the opportunity to experience the benefits of recreational horseback riding, and reimbursement to families for goods and services that are not funded through other resources such as insurance or Medicaid. Services are also available to individuals with autism spectrum disorder. No fee. Call for an intake appointment. Eligibility: Must reside in Steuben County. Ride with Pride and reimbursement are available to residents of Chemung, Schuyler and Steuben Counties. KIDS ADVENTURE CLUB - Offers a before and after school child care program providing interest based learning centers, board games, group games, sports, dramatic play, art, science, music, books, outdoor play, and quiet play. NURSING HOME TRANSITION AND DIVERSION (NHTD) - Helps individuals transition out of a nursing home setting and return to their home and helps prevent placement into a nursing home. Set fees. OFFICE FOR PEOPLE WITH DEVELOPMENTAL DISABILITIES (OPWDD) DAY SERVICES - Provides day services based on individual needs with services including training in life skills, socialization, prevocational and sensory motor skills, speech, occupational and physical therapy, psychological services, social work services, and nursing services and nutritional services. All day services offer individualized person-centered approaches to training and involvement in the community. Fees vary. Call for an intake appointment. Eligibility: Must reside in Chemung, Schuyler, Steuben, or Tioga County. Must be 18 or older and have a documented developmental disability. OFFICE FOR PEOPLE WITH DEVELOPMENTAL DISABILITIES (OPWDD) RESIDENTIAL SERVICES - Provides residential options designed to meet the individual needs of those with developmental disabilities through a Community Residence (CR), three Intermediate Care Facilities (ICFs), and four Individualized Residential Alternatives (IRAs) with one home specialized to children with autism spectrum disorder. All homes provide 24 hour staffing, room and board, training in all aspects of daily living, nutritional services, nursing services and access to medical and additional clinical services. Fees vary. OFFICE FOR PEOPLE WITH DEVELOPMENTAL DISABILITIES (OPWDD) WAIVER SERVICES - Provides alternatives to traditional programs based on needs, interests and desires of each individual including day habilitation, respite, environmental modifications, and adaptive equipment. Must meet OMRDD eligibility criteria as well as Home and Community Based Services (HCBS) Waiver eligibility requirements. OFFICE OF MENTAL HEALTH/HOME AND COMMUNITY-BASED (OMH/HCBS) WAIVER SERVICES - Provides an individualized care approach to children and adolescents at risk of needing institutional level of care. Services are based on needs and interests of the family and may include individualized care coordination, 24-hour crisis response, intensive in-home services, respite care (hourly and overnight), family support and skill building. Fees vary. Call for an intake appointment. Eligibility: Must reside in Chemung, Ontario, Schuyler, Seneca, Steuben, Tioga, Tompkins, or Yates County. Must be 5-18 years old, have a viable family willing to participate in services, and meet the clinical requirements for serious emotional disturbance (SED). PATHWAYS TO LEARNING DAY TREATMENT PROGRAM FOR CHILDREN AND YOUTH - Provides intensive mental health services to school-aged children and youth within the educational setting. Students receive individual and group therapy in conjunction with special education programming in a 6:1:1 classroom setting. Other services include family therapy, clinical care management, medication therapy, and socio-recreational therapy. Program is a collaborative effort between Pathways, Inc. and Steuben-Allegany BOCES. The New York State Office of Mental Health licenses the program. No fee. Call for an intake appointment. Eligibility: Must be enrolled in East High School, Northside Blodgett Middle School or Kent-Phillips Elementary School located in Corning; or the Hornell Elementary, Intermediate and High Schools. PRESCHOOL PROGRAM - Services provided are determined by each child's Individualized Education Plan (IEP) including special education, speech therapy, occupational therapy, physical therapy, counseling, nursing, one-on-one aides, sign language, and augmentative communication services in addition to the structured and unstructured learning centers that are used to develop improved learning, fine motor, gross motor, language, and social skills. Programming is for three and four year old with disabilities. REACH FAMILY BASED-TREATMENT PROGRAM - Provides surrogate foster care for children with serious emotional, social, and behavioral problems that are assessed to benefit from out-of-home placement. Trained Resource Parents provide care in their home for the youth with a team of professionals to support the youth, their family and the Resource Family. Set fees. Call for an intake appointment. Eligibility: Must be 5-18 years with serious emotional, social, and behavioral problems that are assessed to benefit from out-of-home placement. SERVICE COORDINATION - Provides linkage and referral to supports and services for individuals with developmental disabilities or a traumatic brain injury to allow them to live as independently and productively as possible. Service Coordinators act as advocates and service liaisons to navigate the service delivery system with individuals receiving services and assist with securing Medicaid and with the application process for most supports and services. TRAUMATIC BRAIN INJURY (TBI) SERVICES - Provides supports and services through the Department of Health Traumatic Brain Injury (DOH-TBI) Waiver Program including service coordination, Home and Community Support Services (HCSS), Independent Living Skills Training and Development (ILST&D), respite care primarily within the client's home, environmental modifications to ensure the health, welfare, and safety of the individual, or to enable the individual to function with greater independence in and around the home and transportation services. Must have be diagnosed with Traumatic Brain Injury (TBI), 18 or older and meet requirements through the DOH-TBI Waiver Program. YOUTH WRAP AROUND ALTERNATIVE PROGRAM (WRAP) - Provides short-term therapy in order for a child that has been in placement for some time transition back to their families and communities, as well as to their home schools. Services may include initial strength-based assessment, coordination with other agencies, home and community support services, family, individual, and group therapies, crisis intervention, school supports, crisis visits, and social skills training, and respite services.
